Unique Families Support service
A fully funded specialist family support service for families in Stoke or Staffordshire who have a child or young person with SEND or mental health needs.
This service is currently in development phase. We are looking for families who would be happy to be the first to receive support from a SEND Specialist Family Support Worker, and who would be happy give us feedback on our service so that we can shape and develop our specialist family support offer.
What does this service offer?
Practical and emotional support which is bespoke for the family, but may cover:
- managing emotions and behaviour
- problem-solving sleep and eating issues
- managing change and transition
- issues in education, health or social care
- finding and accessing support services and social opportunities
- difficulties accessing or maintaining employment
- financial and housing matters
Eligibility
Families should have:
- at least child or young person aged under 25 with SEND (no diagnosis required)
AND at least one of the following:
- difficulties across multiple areas of family life
- school aged child with attendance difficulties
- school aged child at risk of suspension or exclusion
- young person aged 16-25 with SEND or mental health who is NEET (not in education, employment or training)
- parent/carer/sibling who has mental health needs, physical health needs or disabilities

BESPOKE PACKAGES OF SUPPORT
All packages are bespoke dependent upon need, but may include SEND needs screening, liaising with services, multi-agency meetings, referrals, support with the EHCP process, child wishes and feelings work, following appeals or complaints processes.
- Emotionally Based School Avoidance (EBSA) support package

- Attendance at school meetings in person - £60 plus travel expenses from Burntwood at 45p per mile

- Attendance at mediation meetings - £50
- SEND Tribunal representation: an appeal against a refusal by the local authority to assess for an EHCP - £150
- SEND Tribunal representation: an appeal about the contents of an EHCP (up to, but not including hearing day) - £250
- SEND Tribunal representation: any other type of appeal (up to, but not including hearing day) - £200
- SEND Tribunal representation: hearing - £100
*Draft EHCP checking is time-limited as you are only given a 2 week window to provide feedback to the local authority.
